I have spoken to our billing office in Fort Worth, Texas and below is a summary of this account: On January 3, 2016, we reimbursed patient’s credit card two payments in the amount of $150.00, ($300.00).On January 29, 2016, we reimbursed patient’s credit card a payment in the amount of $150.00.On February 12, 2016, a check in the amount of $119.73 was mailed to the patient, (ck#17005). We have reimbursed patient’s credit card $450.00 and we have mailed a check in the amount of $119.73 to the patient as well.  This amount totals $569.73 in reimbursement to Ms. [redacted].
