I am rejecting this response because: I received a cd with 3 doctor's visits from Dr. [redacted] in the seven years he was my primary care physician. One report in 02/2011, 01/2009, and 12/2009. I recently received the physical therapy reports from an office that Dr. [redacted] reffered me to; his name is on the report, but he did not have that visit listed on the cd that he sent me. I have decided to get my medical records history from the insurance company where Dr. [redacted] filed for payments for services rendered. Dr. [redacted] had the opportunity to at least say that maybe he made a mistake and didn't send all of the records to me. He chose to say that my claims are unsubstantiated. The medical reports that I'm receiving from my insurance company will show that I visited Dr. [redacted] office more than three times in the course of a seven period from 2007 to 2014.
