We have the registration form at our store. We've had the form for you for several weeks now. Our store manager has left you several messages about the registration form on the number you left us but haven't heard back from you. It's as simple as picking up the form and registering your puppy with the ACA or the AKC under their Canine Partner Program with the ACA or both if you like.
