In response, our office staff is laid off and last day worked was April 8th, so this is the first opportunity to respond. We currently are in and out of office at least a couple times per week to check messages, mail and emails. Upon checking personal info from file, completion...

paperwork was faxed on two different dates; 3-31-15 and 5-20-15. We do not have any record of this student calling 93 times. This student left one voicemail and the information that was requested, was faxed.
