Good Afternoon,   After reviewing the account it does appear that [redacted] has contacted our office several times stating that his insurance had made payment.   We did receive a copy of the patients explanation of benefits, the patients insurance was contacted at that time...

and a check was reissued.   Due to us not receiving the payment statements were issued out to the patient’s home address. The patient then contacted out office again and was informed we never received the payment from insurance.   We have contacted the insurance and verified  that the check was submitted but not received by our office, the insurance will resubmit the check to our office.   Once the payment is received the patient will have a zero balance on his account with us.
