Pursuant to your conversation with Irina here at the office I am answering a complaint letter under ID# sited above. We have been in communication with the patient over this issue for weeks already. Patient's records contain full set of blood work results performed for...
her on the day of her visit on 3/**/2015. Patient was seen by the doctor who ordered blood work for her, we are prepared to provide medical documentation for review to show what exactly was done for the patient. Patient's insurance company, Empire [redacted], issued some checks to the policy holder on the patient's account. We only requested that the money received for provided services would be forwarded on to us. We did not request that any out of pocket payments should be made by the patient. According to Anshu at the insurance company three checks were issued as follows: $514.14 on 4/**/2015 ref# 071515-006090 $432.00 on 4/**/2015 ref# SAA $591.20 on 4/**/2015 ref# [redacted] We assert that no fraudulent activity has taken place and that everything was done according to the rules and regulations. We also are willing to give the patient the benefit of a doubt and assume that she misunderstands the situation. However we will not expunge the charges and will continue to pursue reimbursement. Thank you for your time and attention to this matter, Sincerely [redacted] Billing and Collection Department ###-###-####
