We have resolved this issue with the claimant, [redacted].  The customer Service manager called and took his information over the phone as a one-time courtesy.  The reason this wasn't done by the customer service rep is because his plan's subrogation clause requires this information...

EE SAID THAT HE REC'D AN ACCIDENT FORM AND RETURN ENVELOPE BUT IT WAS NOT PRE PAID. HE SAID THAT HE SHOULD NOT HAVE TO INCUR ADDITIONAL CHARGES IN ORDER TO RETURN THIS INFO. SO HE REQUESTED THAT I TAKE INFO OVER THE PHONE.
 
ADVISED EE THAT I AM NOT ABLE TO TAKE INFO OVER THE PHONE THIS IS A REQUIREMENT. EE CAN GO TO www.pbaclaims.com AND SEND IT ELECTRONICALLY.
 
HE ASKED WHAT WILL HAPPEN IF HE DOES NOT SEND INFO BACK
