February 16, 2016To Whom it May Concern:Please be advised we just received this letter via email from the clinic where the patient was treatinglooked into the account for [redacted] and we have no log of [redacted] 's callsHowever we had a patient representative who was taken out of the office by ambulance, she was the last person in his account in October 2015, and is not returning due to medical problems[redacted] presented his Personal Choice insurance card at the time of his first visit and was paying toward his out of pocket expense, After his treatment ended it was discovered that [redacted] had Medicare as a secondary which did not pay all of his coinsuranceTherefore, [redacted] 's refund is $122.57, he did pay a total of $but we used part of it to satisfy his balance, we have taken care of this and it will be issued to him immediately.Sincerely,Theresa R
October 9, 2017 Dear [redacted], We are writing this letter in response to complaint [redacted] from [redacted]. When a patient is referred for physical therapy it is our policy to verify the insurance benefits and review with the patient at the time of the initial evaluation. [redacted] had...
a high deductible plan, it is not legal to offer a selfpay rate when a patient has health insurance. We reviewed the benefits with [redacted] (attached is the coinsurance form signed by [redacted]) and our policy to collect $20 per visit to go towards her deductible. I apologize if there was a misunderstanding regarding our policy of collecting the $20 to go towards her balance as payment in full for the services. I have also include the financial statement and face sheet that were signed by [redacted]. These forms also explain that any amount not paid by the insurance carrier is the patient's responsibility. [redacted] did receive the services and her insurance was billed. The insurance company has applied these services to her deductible.We have reviewed with the staff the importance of clearly explaining that the $20 that is collected is going towards the patient's balance, not payment in full for services so that there is no misunderstanding.
