The patient from case ID [redacted] was seen in our office on May 16, for an examination At this visit, a Cone Beam CT was taken and read by our Oral and Maxillofacial Surgeon for diagnostic purposes The patient was informed of the costs of the scan and agreed to such The patient decided not to move forward with treatment, at which time, her visit was resubmitted to the patient's insurance company as a corrected claim for a consultation rather than a limited exam The patient did pay her copayment amount for the consultation however she did not meet the financial responsibility of her CT as dictated by her insurance company (and communicated as such to the patient directly via her Explanation of Benefits) All coding and collection fees was done so in accordance with the ADA guidelines as supported by the Missouri Dental Board decision dated November 3, We regret to inform you that we will be unable to assist further with this matter

We called [redacted] and after being very rudely addressed, got them to agree to pay for the 0120 on the 8/24/2015 under the wife's policy as secondary. The secondary had paid $0 on the 1/21/15 (primary paid $40), and the secondary had paid $15.20 on Dr. [redacted] on 5/8/2015 (primary paid...
$60.80). Their system incorrectly denied the 3rd date of service because it saw 2 previous [redacted] during 2015. But they should not have denied the 8/24 one because the secondary paid $0 on the first one of the year.
The patients insurance incorrectly denied the claim, but we have appealed it on his behalf and that they are sending it through for reprocessing.
