home address from our bill pay service in Utah. We are very sorry that this has occurred. Despite the best efforts of our office there are inherent delays that occur when dealing with the insurance companies. To elaborate would be an exercise in futility. Furthermore, we would like to express that Mr. Pethtel’s removal from the practice was completely unrelated to this billing matter and based solely on interactions between Mr. [redacted] and the staff. Based on his comments to staff members, management felt it was best he receive care elsewhere. Trust is a vital part of any health care relationship and Mr. [redacted] should enjoy that relationship. We apologize for any delay in this refund and appreciate this civil forum to rectify such matters. My absence due to a vacation (the reason I filed for an extension to this response) and subsequent catch up maybe partly to blame as well.
