On 12/28/16, the following documents were sent on our behalf in regards to complaint ID# ***, *** ***.This member claims to have sent a letter cancelling her membership in SeptemberAt this time Ms*** was not eligible to cancel because she has not completed her month commitment.The
membership suspended her membership for health reasonsHer membership freeze ended 10/01/We attempted to bill Ms *** for her monthly duesAt this time her card on file declinedHer card continued to declined for November and DecemberOn 12/05/16, attempted to charge the card and the payment was successfulAt this time the Mr*** was delinquent for months worth of dues plus the maintenance feeAlso, she has occurred late fees for a total of $156.48.Although this Ms *** was in a committed membership until 2/01/2017, we as an act of good faith, cancelled her membership effective immediately and we gave her a partial refund of $for the late fees occurred.Please see Ms *** membership agreement and addendum she signed.Thank you so much for your help! I really appreciate it.

To whom it may concern,I am writing in regards to the recent complaint that we received from our member, Ms *** ***.Ms*** signed a month membership agreement in 10/09/During the time length of her commitment, Ms *** was put on a frozen status for medical reasonsThis extended
her commitment length until 2/01/I personally spoke to Ms *** in June and explained that her membership was going on a suspended status until 6/30/and then will get billed as starting 7/01/Ms*** is not eligible to cancel at this and we did not receive a written request for cancellation as per our policy dictatesIn October 2016, the member's credit card declined for her monthly dues as it did again in November and DecemberHowever, we continue to attempt to retrieve payment until days of past due balanceIf the account is delinquent past days, the account is sent to a collection agency in attempt to reconcileOn 12/09/16, Ms *** payment was successfully recovered from our payment systemThis amount of $is for the following: October, November and December dues of 21.35, the annual maintenance fee of $and billing return fees. As per the General Manager and myself, we would like to extend the following offer to Ms ***We would like cancel her membership effective immediately and refund $for the late fees to her credit cardTherefore, Ms *** will not receive any further charges from Quest Fitness and will be terminated right away even though she is still committed for another monthsWe are offering this compromise to the member as a token of good willPlease have Ms *** reach out to me so we can discuss this furtherThis is the 1st time that I had been made aware of this issue and we are more than willing to work with her in order rectify any discretion she has toward our clubI have provided you a copy of her membership agreement and of the addendum she signed agreeing to the annual maintenance fee charge Please see section of the membership agreement that will explain our cancellation policy.Thank you and please let me know how to proceed.Sincerely,*** ***Billing Manager
