Shrewsbury Ambulatory Anesthesia bill has been paid 201, I have the records to prove it. I have they exact dates when they were paid. this information can be forwarded if needed.
While *** has advised our office that he had paid the outstanding debt, no proofs of payment were provided. We have tried to contact *** on several occasions to discuss this matter with him, however, he has not responded to such attempts. If Mr. Gilmore can contact our office at ***, we can try to resolve this matter.

This will be our last response as we are not going to engage *** in this frivolous back and forth, especially as he has now resorted to making false accusations against our firm. The fact is that he has not provided proof of payment. We sent *** a letter on January 19, 2018 advising him of the outstanding debt. We received a response from *** dated May 18, 2018 which contained a document claiming dates that the debts were purportedly paid, but providing no actual proof of payment. So we have the dates that *** claims payments were made, but he has never provided proof. Our client has received payments for other days of treatment for ***, but not the two dates which are the basis for the outstanding debt. Thereafter, we tried reaching *** by phone on several occasions (we have not been authorized to leave messages), sent *** a letter on April 23, 2019, and then another letter on September 23, 2019, asking that he contact us. (Thereby refuting his claim that this is the first notice he received, we have a copy of his May 18, 2018 response.) Instead, he chose to file this complaint. We are a law firm and have been trying to resolve this without litigation. If *** will not contact us to discuss what he may be able to do with respect to his insurance company, then he leaves us no choice but to move ahead accordingly.
