The procedure leading to the decision is. On June 12,2016 I was told that my carrier  would  pay $444.44. and I was  given a billed for $444.44. I find it unbelievable that they would issue a billing statement stating  the above amount and then on July 12,2016, When I thought I was making  my final payment,  I was then informed that the carrier had disproved the claim. Why was I told it was approved on June 12,2016? As for verifying my coverage Are you saying I shouldn't had believe what what you stated on june 6,2016?**
