I received my first claim notice on 08/12/99. I spoke to Polk County EMS and it was determined that the billing was in error. I was told the claim
I was told in speaking to both the billing for Polk County Bocc EMS that the orginal claim was made in error and would be removed. I received another notice and I now have 4 claims. Claim# ***, Claim # *** Claim # *** Claim#***. I live in PA and do not come to Fl. How is this company using my personal information and how can I get it stop? I have left messages with both Polk County BOCC-EMS at 1-888-987-6918 and UHC.
I want this to stop. If a person has my information, I need to know why this company did not stop billing in 2019? Why has this been allowed to continue.

I am a representative of the billing company, R1, that Polk County has contracted with to do their EMS billing. The client forwarded the complaint information to me. We are unable to identify this patient with the information provided. We have several patients by this name but none with an address in PA. We had received information from you with what appeared to be an email for the patient. We have reached out by email with no response. We also received a phone number and that phone number does not appear to be any good. We want to be able to respond to the complaint and address any issues but we need to be able to identify the patient. Please provide additional information or have the patient contact me directly so that we can address any issue that she may have.

I spoke to their manager and provided the additional information that they requested. Their manager confirmed that she could see the original notes plus the three additional charges. Response from UHC said that the company needed to confirm with them and that the charges was made in error and they would correct my claim history. They could not take the info from me . The manager stated that she was noting the account and sending it to the director. All correspondence must come from the director not the call center manager. They do have my email and my phone info as they called me at 8pm.
We were able to identify the billings in question based on the information that was emailed to us by the consumer. Unfortunately her insurance was billed in error based on a name and DOB match with another patient for the client with the same name and DOB. We perform routine eligibility checks with numerous large payers. Unfortunately United Healthcare will return a positive result based on name and DOB only, without a match on the SSN. We have notified our eligibility clearinghouse about this several times and have once again mentioned it to them as a result of this case. Also United Healthcare has acknowledged that there is issue here as well and are working on it. In the interim we have implemented mitigative efforts on our side to avoid this situation and have those accounts sent for further manual review and verfication before relying on those partial matches from United Healthcare. There was one of the 4 accounts that got billed to the consumers insurance that paid in the amount of $32.82. That amount was submitted today for a prompt refund to be processed back to United Healthcare. Also, the consumers insurance has been removed from the account. We hope that this explanation satisfies the consumers concern and again apologize for the inconvenience caused by this issue.
Have insurance; over charged me when I used Polk County EMS. I have copies of my EOB which I should only be charged $364.69 but Polk County seems to think I owe them $614.00. Polk County stated that the insurance needs to update their records and they refuse to comment on the issue with the State of Florida Consumer Services. Consumer services closed the case because Polk County had no response. I found a Florida Insurance booklet and it clearly states that insurance dictates the charges, not the provider. I'm getting nowhere with this so it's time to call the TV stations and get them involved.
