Per our 3rd party Billing Company.
 
We have had the account placed on hold since June [redacted].  We are having difficulty locating the payment and cannot get confirmation from [redacted] as to where the payment was sent.  We have not sent a statement to the patient since June of [redacted]...

and we told her when she called that the statement was sent in error.  Our issue is with [redacted] and we will not be billing the patient.
