You purchased the insurance. Your insurance dictates the fees we have to charge. You signed that any copay quotes we give are just estimates. I can send you the signed consent. The insurance is a contract between you and your insurance. We collected upfront and you signed the consent so you were aware of all of the fees due prior to treatment. In this case, you knew the cost, accepted the treatment and paid before the treatment started. We are not understanding why you are upset with your insurance rates. Your insurance already gave you a discounted rate. We only verify that you have insurance. You are responsible for knowing the details of the insurance plan you purchased.

A refund has been issued for the work paid for and not received. I have a copy of the refund receipt on file. The refund amount was put back on the patient's Care Credit account as requested. We believe the issue has been resolved.

While Ms. [redacted] paid $855.00 for her procedure, she was refunded $630.00 on 22 December 2016. The balance of $225.00 was not refunded to her and will not be as that is her insurance co-payment for that procedure per the attached co-payment chart. If you would reference procedure code D3330, you can see the the patient co-pay amount due from the patient is $225.00 (which is the difference of $855.00 and $630.00). Thank you!
