The total claim was $293.00. Ms. [redacted]'s insurance [redacted] paid $139.00 on 8/27/14 for date of service...

7/24/14. A small balance of $5.00 was left on the account and was courtesy adjusted off on 9/4/14. I apologize if there was a misunderstanding that Ms. [redacted] was to receive a credit.
