Piedmont Mountainside Hospital

Piedmont is cashing my checks, but still turned me over to collections and ruined my credit
I received care at Piedmont Hospital (and ASSOC IN DIAGNOSTIC RADIOLOG inside the hospital) back in I am a farmer with varied income because of seasons and weatherI don't have and cannot afford health insurance, so was provided the Financial Assistance forms in July 2013, and submitted the forms per the instructions
I received confirmation of the forms being received (I have documentation to support) on 7/
In August of that year I hadn't heard back from the facility and contacted them and was told they were extremely behind and would let me knowI asked about the Bill and they said it would be fine
I followed up again in September and they said they didn't receive the forms and had no proof I sent it inI then faxed it to them, along with the original submission via email and proof that they received itI kept copies of the faxes I sent in and the printout showing
