On or around May 4, 2015 one of the reps from [redacted] Health Services spoke with Mr. [redacted] about receiving his medical supplies. Our records show that Mr. [redacted] agreed to the service and the account was then sent overt to [redacted] Pharmacy to fulfill the order for Mr. [redacted]. Our records do...

indicate that on 8/25/15 we received a shipment back from Mr. [redacted] and at that point one of our reps reached out to him to see if it was an address issue or if he returned the item himself. Mr. [redacted] stated that he was not longer in need of the items and would like to have his account placed on hold. The shipment was credited to his account on 8/25/15. As of yet, we have not received any other shipments back from Mr. [redacted] to be credited. As of today the account for Mr. [redacted] has been canceled and he has been placed on our Do Not Call list. We apologize for any inconvenience we may have caused Mr. [redacted] and feel the steps have been taken to give him the desired settlement that he has asked for.
