02/20/2017Dear Cynthia,We did bloodwork for [redacted] on 6/23/2016. All tests were run at Perll Diagnostics, Inc. except for the Lyme Antibody which was referred to [redacted].We submitted a claim for the tests run at Peril Diagnostics Inc. to [redacted]'s insurance which at the time was...

We did not charge for the test referred to [redacted].Loomis sent us an explanation of benefits (EOB attached) which shows it paid partially and shows a patient (member) responsibility of $187.20.
