Unfortunately, our staff entered his insurance information in correctly, which has since been corrected.  The back dated authorization (appeal) was done on 11/3/2016 and the patient has been seen by the specialist again since that date.  I spoke to the specialist office on 12/20/16...

and they rebilled his initial office visit on 12/12/16.  I'm not sure why United Health Care told him there was no "appeal" in his file, as it was approved on 11/3/16.  I have spoken with the specialist office and Mr. Cheng himself and have apologized and asked him to contact me directly if he had any other issues with this claim or our office.
