They failed to mention that my dental provider was now out of network prior to performing services even though I filled out paperwork upon arrival.
On August 5, 2019 I went to Peach Tree Dental in Ruston, LA and was required to refill out information for my medical history. I asked if I needed to provide my insurance information again and was told no, that it was still on file. I was not informed at that time that my insurance was now out of network for them. My teeth were cleaned then I was told that I would need a "deep cleaning" that would have to be set up on a different day. I spoke with a lady in the front office that provided me with pricing for the "deep cleaning" of nearly $400.00 after insurance coverage: note I was still not informed that they were no longer in network. That pricing did not seem correct as my wife had the same cleaning just a few months prior for approximately $180.00. I gathered paperwork/billing information from my wife's cleaning and returned to compare and find out when I presented them with the difference in pricing that they were "no longer in network."
Per my insurance information, which was updated with correct "in-network" providers as of 8/12/2019 Peach Tree Dental is still listed as a network provider.
