Review: I paid for architectural drawings to apply for a permit for the deck and the roof over the deck. The elevation form requested by the Village of Valley Stream was requested on Nov **, 2013. [redacted] said she would provide it as part of the $1,000 that was paid but did not. She does return any phone calls to me, my attorney, the Village Engineer. I need this form in order to close on the selling of the house.Desired Settlement: complete the elevation form and submit it to the Village of Valley Stream as soon as possible
