I would like to address this issue as our practice has gone above...
and beyond for this patient who has filed this complaint. This patients complaint begins with stating that an incorrect medication was prescribed that he cannot take. His chart indicates no drug allergies. On August 21, 2013 he was prescribed Levaquin for Klebsiella infection and Prednisone taper for Tracheobronchitis. Patient was also diagnosed with a lung nodule. Patient called our office on August 27, 2013 stating that he cannot take Prednisone and asked for a sleep aid instead. This request was discussed with the Doctor in presence of office manager. Patient was advised at that time that he could have a five day supply of sleep medicine if the sleep disturbance is related to the cough. He was told that he would need a sleep evaluation if it is not cough related and that any further sleep medication would need to be prescribed by his Primary Care Provider. He stated he would be seeing his PCP the next day and would discuss it there. Five tablets of Lunesta 2mg were called to patients pharmacy. On September 3, 2013 patient called again and was directed to the office manager. After speaking with the patient, he notified the office manager that it is NOT a cough that is keeping him awake and that he believed it to be anxiety. He states his PCP was ordering radiology of his head for evaluation and that he would be following up with them the next day for results. Multiple notes were made in this patients charts due to his continuous calls to our office, all of which were answered or returned. He continued to call the office for more antibiotics in which he was told he would need to be seen and evaluated. Patient canceled or no-showed many times. He did not agree to come back into the office until his visit on December 2, 2013 in which he was seen by Dr. [redacted]h but was spitting through the entire visit and had written on a piece of paper that he lost his voice and could not speak to us. He has only been seen in our office three times. From the first visit he started requesting disability paperwork to be filled out. We informed him that our office would fax all records pertaining to his care to Prudential and that his PCP would need to coordinate his disability paperwork. He was advised to see an Ear, Nose and Throat Specialist as well as an Infectious Disease Specialist as his symptoms were not consistent with a lung nodule. He agreed to see another doctor and his records were faxed to expedite the process. Patient began calling again requesting that we call Prudential regarding his disability. Records had already been faxed to Prudential from our office and [redacted], a medical assistant in our practice, called Prudential as patient requested. I have attached all documentation pertaining to this response. We will be happy to assist patient with a copy of his records should he choose to see a different pulmonologist.Sincerely,Office Manager
