Mr [redacted] came to our practice on 5/7/The doctor's comprehensive exam and X-ray revealed his tooth #is fractured on the distal lingual surfaceMr [redacted] was presented with this chief concern of fractured UL tooth and the doctor recommended a buildup and crown to save the tooth Mr [redacted] declined the doctor's recommendation for the crown and opted only for the buildup and that is what the doctor did on that dayAlso the periodontal evaluation revealed that Mr [redacted] needs a deep scaling & root planning and he was advised of that No teeth cleaning were done on Mr [redacted] Hence, Mr [redacted] was never billed for any teeth cleaningTherefore, we are at a loss why Mr [redacted] is complaining about "Dental cleaning".Our patients are made aware that we contact and bill their insurance as a courtesy to them; as is the case with all dental and medical practicesMany times the information the insurance company provide us is inaccurate, outdated, or a rough estimate at bestAnd many times the insurance company does not pay our office due to a number of reasons, including incorrect information, insufficient time for insurance benefits, etcIt is also the patient responsibility to follow up with their insurance.The bill that Mr [redacted] refers to covered the services that were done on him and his son and it did NOT include any charges for cleaning! In addition to the comprehensive exam, X-ray, and buildup that were done on Mr [redacted] , his son had fillings and sealants done on his teeth alsoWe have arranged more than once for the doctor and my manager to sit down and discuss the issue with Mr [redacted] , but each time Mr [redacted] fails to show up to the meetingWe are willing to discuss all available options with Mr [redacted] to resolve this issue.Sincerely,Emily S [redacted] Office Manager(303) 857-[redacted]

Mr. [redacted] declined the doctor's recommendation for the crown and opted only for the buildup and that is what the doctor did on that day. Also the periodontal evaluation revealed that Mr. [redacted] needs a deep scaling & root planning and he was advised of that. No teeth cleaning were done on Mr. [redacted]. Hence, Mr. [redacted] was never billed for any teeth cleaning.
