January 18, 2016To whom it may concern,We are writing in response to the complaint filed for treatment rendered for our patient, [redacted] ***We have reviewed the account, and are unable to give the patient's guardian any reimbursement; as the charges are accurate.When the patient was seen for their routine examination and hygiene appointment, it was determined that the patient would benefit from sealants, to prevent decayThe patient's guardian was informed that her basic breakdown of insurance (which is performed as a standard courtesy, for all of our patients, not out of requirement) indicates that her preventative and diagnostic services would be covered at 100%As much as we would like to be aware of every clause and limitation of every patient's individual insurance policies, it is not a feasible possibility.It is unfortunate the patient's insurance policy denied a portion of her claimAll of our patients sign an agreement as a new patient; that they understand that insurance claims are subject to review, and if their insurance denies their claims, they are liable for the balanceA copy of the patient's signed document is enclosed for reference.Please let us know if you require any further documentation or we can be of further assistance in this matter.Sincerely,Morgan TOffice Manager
