Good Morning:PAH received the hard copy files from [redacted] on August 3rd.We were informed by the Regional Representative of National Veterinary Associates (NVA) [redacted] that all patients had been sent a letter and/or...
an email informing them that the practice was closing. [redacted] also stated that PAH would receive the Avimark electronic files for [redacted] as [redacted]s files from 2015 on were all electronic. As of August 25,2015 PAH has not received the electronic files. PAH attempted to satisfy all patients that asked for hard copy files that we had in our possession (many files were missing) At this point, NVA has all electronic files located at the parent corporation in California. I attempted to obtain all electronic files that were requested from [redacted] clients. Many times, my email requests were not answered. At this point, in order to expedite claims I am requesting that patients contact [redacted] directly at: [redacted]. to obtain their electronic files. Records for C[redacted] were picked up from PAH on August 11, 2016. We have no further records or information. [redacted] would need to email: [redacted] to obtain the electronic files for [redacted]. PAH does not, nor did PAH ever have them in our possession.
