Review: I have been trying since 2013 to present to get complete dates of Therapy from 2013 to March 2014. He does not respond to Phone Calls, E-Mails or Certified Register mail, he signed for it August 15, 2014.. It is imperative that I also have the complete amount I paid him for Therapy, so I can be paid back by the [redacted] Fee Basis Office for Authorized and Approved care for conditions related to my Service Connected conditions from injuries in the [redacted].Desired Settlement: I need above paperwork and final diagnosis and care received to get paid for.
