On August 2, the office went over Co-payments with *** one more time and she signed another treatment plan sheet along with Consent forms to do treatmentPayment of 5755,was collected and all services were rendered for payment that day. On September 22, *** called Angry and was being verbally Aggressive towards the office*** was accusing the office of overcharging herI explained that according to her Insurance plan her Co-payments were correct but I would call the Insurance Company to double Confirm, On September 22, at approximately 3:pm I called DeltaCare USA at ###-###-####, I spoke to Representative *** *and she said all charges were correct and that she sees in her computer that *** *** called that same Morning of September 22, Representative *** *** confirmed that all charges were correct and *** was billed correctly. The charges are correct and it was verified with DeltaCare USA at ###-###-####, Enclosed you will find all Documentation was agreed and signed.
