The complaint was filed against Oral & Facial Surgery Associates by our patient [redacted] [redacted] had an extraction and bone graft done on 8/21/and an extraction and implant placed on 12/12/by DrThomas SchugelWe sent a claim to his dental insurance ( [redacted] ) on both days of surgeryWe received an explanation of benefits form [redacted] on 1/06/asking for us to send them an x-ray for date of service 8/21/That was sent out that dayThey then paid $and we had a write off of $We then sent [redacted] a refund check of $on 1/21/On 1/26/ [redacted] asked for the x-ray for the implantWe also sent this off to them again on that dayWe received a letter from [redacted] on 2/26/asking for the refund for the implantAt that point we had not received any additional payment from [redacted] , so there was no refund to send himWe got a payment from [redacted] for the 12/12/date of service on 3/5/for $and a write off of $A refund check of was sent to [redacted] on 3/16/As you can see we sent the refund checks to [redacted] I within business days from when we received the payment from [redacted] I would appreciate if this case does not get published on your website to harm the reputation of the practiceWe always send x-rays to [redacted] with claims because we know it is something that they need to pay claimWe do everything we can to get claims paid quicklyI seems that [redacted] ’s issue is with his insurance and not us
The complaint was filed against Oral & Facial Surgery Associates by our patient [redacted]. [redacted] had an extraction and bone graft done on 8/21/14 and an extraction and implant placed on 12/12/2014 by Dr. Thomas Schugel. We sent a claim to his dental insurance ([redacted]) on both days of...
surgery. We received an explanation of benefits form [redacted] on 1/06/2015 asking for us to send them an x-ray for date of service 8/21/14. That was sent out that day. They then paid $291.20 and we had a write off of $120.00. We then sent [redacted] a refund check of $236.80 on 1/21/2015. On 1/26/2015 [redacted] asked for the x-ray for the implant. We also sent this off to them again on that day. We received a letter from [redacted] on 2/26/15 asking for the refund for the implant. At that point we had not received any additional payment from [redacted], so there was no refund to send him. We got a payment from [redacted] for the 12/12/14 date of service on 3/5/2014 for $1069.80 and a write off of $37.00. A refund check of 1106.80 was sent to [redacted] on 3/16/2015. As you can see we sent the refund checks to [redacted]i within 7 business days from when we received the payment from [redacted]. I would appreciate if this case does not get published on your website to harm the reputation of the practice. We always send x-rays to [redacted] with claims because we know it is something that they need to pay claim. We do everything we can to get claims paid quickly. I seems that [redacted]’s issue is with his insurance and not us.
