This is in response to a complaint filed by Ms. [redacted] on July 29, 2015. Ms. [redacted] did have a surgery to remove her wisdom teeth performed in our office on January 5, 2015 in our location at [redacted] with Dr. Byron T. H[redacted].Upon checking in for surgery. Ms....

[redacted] requested that our front desk complete the physician section of her FMLA/STD claim for her employer. This was done and, as a courtesy to the patient, we faxed the paperwork to the number provided. These forms are frequently provided to us by employees, normally in advance of their surgery, and are routinely completed by our office.Our front desk personnel were made aware by the patient that the fax transmission was not successful two days later. We refaxed the paperwork at that time.In early March, Ms. [redacted] contacted us to advise that because the fax was not successful on the day of surgery, her FMLA/STD claim had been denied by her employer, and that she wanted us to pay her wages. But since we are not a party to the employer claim and/or pay policy, we believe that the responsibility for insuring that employer claim requirements are met belong to the employee.Since that time, Ms. [redacted]'s dental insurance policy has settled her claim, leaving a balance of $186.00 on her account. While we do not feel we are responsible for the patient's pay while on leave from their employer, we do feel badly that Ms. [redacted] is unhappy with our office and on August 3, 2015 we wrote off her a balance.Ms. [redacted] has not been sent to a debt collector by our office, so should not be receiving any calls on our behalf.Please let me know if you have any questions or require additional information.Sincerely,Lyn M.
