that took place. There will be one handler of an issue and the issue will always come to me or in my absence the physician of the patient. The receiver of the issue is to be empathetic to the issue and obtain relevant information. There is to be no comments or insinuations that would create a negative impact. Voices and tones are important. Instead of putting the lab re[redacted] information in the body of the patient demographic sheet - the request is being placed by the patients name. This area of the sheet is most prominent and allows for greater viewing on all involved staff.  The most important outcome was the necessity of strong, kind communication  to the patient and as well as strength of communication between the clerical staff and clinical staff. We thank you for the opportunity to look at this issue as a "case history" and to learn from each aspect of the complaint/ issue and to unfold areas of improvement that will benefit the patients and practice. Regards, [redacted]Manager
