The issue that occurred was that a single page fingerprint request form was accidentally sent, via email attachment, to a student that was originally registered in the same class, that had the same last name That student then called both this office and and the claimant to advise both of the errorUpon this notice, my office requested that the student delete both the email in question and the attachmentThe student stated they would complyWe have since been in contact with the claimant and assured her that if any issue occurred as a direct result of this error that she should contact us immediately and that we would assist in attempting to rectify whatever that situation might be We also advised her that we had already investigated the situation and had already implemented additional steps designed to minimize the possibility that this type of human error would occur in the future

the error. Upon this notice, my office requested that the student delete both the email in question and the attachment. The student stated they would comply.
