Our office has contacted and spoken to Mr. [redacted] on numerous occasion regarding his dispute. He was verbally explained the terms of his prepaid discounted treatment program and recieved a copy of his contract immediately after its initial signing on 08/16/14. Since he decided to discontinue...

his treatment we have mailed him a copy of his contract and patient ledger which shows he had 11 office visits where he did recieve treatment (please refer to the attached patient ledger for a detailed breakdown in services rendered and cost). Regardless of explaining the clear terms of his contract Mr. [redacted] went a head and filed a [redacted] dispute on 12/09/14 and again on 01/22/15 in a attempt to attain a refund for his program fees. Our office won both disputes. He has continually requested copies of our patient sign in sheets witch we can not provide as it is a HIPAA violation.
