Hello Ms***, In regards to Mr*** concerns our Operations Leader has talked with him and he has advised us that his ex-wife is and was covered under his insurance at the time of service. It was also proven that the ex-wife was indeed seen at *** Dental on 5/12/2016, we
have chart notes documented by both the Doctor and the Hygienist. Unfortunately because Mr*** carries the insurance and his ex-wife is on the policy he is considered the guarantor of the account. We have asked Mr*** to confirm the insurance we have on file is correct being the claims are denied, we have suggested that he contact his Human Resource Department/Benefit Coordinator for this information. Mr*** stated that he would contact the Operations leader with this information, and as of today he has not. If it is proven that the ex-wife was NOT covered by Mr*** insurance, then we can indeed change the account information into her name once he provides us with her current address and contact information.Thank you! ***
