Review: I received a call from [redacted] 3[redacted]###-###-#### to discuss a past due account at 10:38 a.m., December 5, 2013. I returned the call at 10:42 a.m. on December 5, 2013, and spoke with [redacted]. [redacted] said that a past due account from a doctor visit to [redacted] at Missoula Foot and Ankle in July 2013. According to [redacted] at [redacted], [redacted] office sent me a billing statement to an incorrect address. The billing statement was returned to [redacted] office due to an incorrect address. [redacted] office sent my bill to a collection agency. I did visit and get a diagnosis from [redacted]. I provided insurance information with [redacted] office and did not receive an insurance billing statement from his office or my insurance provider. I called [redacted] at [redacted] on Friday, December 06, 2013, at 2:10 p.m. to learn the status of her conversation with the doctors office to determine insurance billing.I called [redacted] at [redacted] on Monday, December 9, 2013, at 8:36 a.m. to get an update on the status of doctors billing. She said she left two messages with [redacted] office but did not get an answer. She said she would call again. I asked her to call me after she spoke with the doctors office.I returned the voice mail message from [redacted] from [redacted]. I spoke [redacted] at [redacted] on Tuesday, December 10, 2013, at 2:27 p.m. [redacted] indicated that the [redacted] office submitted the bill to insurance but it was denied.Desired Settlement: I request assistance from the Revdex.com to remedy this situation. I do not contest the bill and willingly agree to pay it. I request to have my bill removed from the collection agency and my credit history repaired to correct any negative credit problems that may affect my current and future credit history and reputation. I dispute the method to collect the payment. Through a clerical error at [redacted] office and no fault of my own, my bill was sent to collection agency, which negatively affects my credit rating and reputation. [redacted] office did have my correct telephone number and insurance provider information and did not contact me via telephone to notify me that their billing statement had been returned due to an incorrect address. I was not aware of the clerical error or the returned bill. I was not notified of the mailing address error and was not provided a reasonable opportunity to give the correct mailing address so I could pay my bill directly to
