show Millennium Pain Management treating this patient. Our records show [redacted] cared for this patient in June of 2015.
If this is the company in question, then our response is as follows. We do have valid Explanation of Benefits (EOB) from Coventry Health Plan (GHP) which shows the patient's responsibility of $269.28 in total. We also show the patient made payments of $224.46, leaving a balance of $44.82. The patient obviously feels he does not owe this, even though [redacted] and GHP both agree to the balance.
Due to the patient's obvious frustration, [redacted] will waive the balance.
Please correct your error in sending this complaint to the wrong company. Millennium Pain Management is not the company in question.
