We went to Urgent Care as the injury was not life threatening but we were pretty sure he needed stitches and we wanted to be sure the wound was well cleaned. We got the bill and we were charged as if we had gone to the emergency room. We were charged $665 for walking in the door(identified as Emergency Care/Urgent Care), $194 for medication (he didn't get any medication), $21.38 for medication/supplies, $57 for Medication/ Devices, $98 for a tetnus shot (identified as Preventive care) and $608 professional service fee. We were also charged $315 for an x-ray, which I do not object to as the cut could have had metal filings in it. For a total of $1,959.08. Insurance adjustments were -$742.21. We paid $232.40 at the time of the service and were told this would be the majority of the bill. I have requested a detailed bill from the clinic. When I talked to the clinic all they had to say was that was their bill.
I cant discuss the patients actual account in detail due to HIPAA, but I am happy to discuss with the patient if they would like more detail. It looks like they have requested a detail bill from the Mercy business office already. Mercy urgent cares are set up as Hospital Outpatient Departments (HOD), which is a Medicare classification. Because we are set up as an HOD, there are two fees; one professional fee and one technical fee. The issue is how the insurance company processes these fees. The professional fee has a copay associated with it. In most cases, the insurance company is applying the technical fee to the patients deductible. This has created challenges in the last few years as insurance companies have increased their deductibles exponentially. As for the concern over us billing them Emergency charges, as you can see below, the Urgent Care is under the Emergency Room classification. We aren’t billing Emergency Room fees. The 456 revenue code is what is on the bill, which is a sub category of the Emergency Room. That’s the revenue code we have to use as per insurance billing guidelines.
