[redacted]'s dog, [redacted], was seen at our clinic on December 24th, 2015 for hindlimb weakness. At this visit, [redacted] was brought in by [redacted]'s significantother. It was suspected that the issue was cervical pain. Radiographs and bloodworkwere recommended and were declined (as noted...

in the medical record). [redacted] wasdispensed two medications for pain control.On December 28th, 2015 our clinic called for a progress report on [redacted]. Her ownersreported that she was doing a lot better, was improving each day and they were abidingby strict rest recommendations.On January 11th, 2016 [redacted] was seen at our clinic to have her ears checked. [redacted]was brought in by [redacted]'s significant other again. It was mentioned that[redacted] was reluctant to climb stairs or jump up. It was suspected that [redacted] hadthoracolumbar pain. Bloodwork was recommended and declined (as noted in themedical record). Pain medications were again dispensed and it was recommended that[redacted] return for radiographs and bloodwork if her signs were not improving. Theowners were instructed to call in 5-7 days with a progress report.Our next communication with [redacted] was on February 5th, 2016 when shecalled to have [redacted]'s pain medications refilled.Our clinic followed routine standards of care for [redacted].
