I have included the advertisement flyer that I received which has the original phone number I called to contact the guy who did the job. I also included the invoice I received from the guy who was onsite who did the job. You can see the difference in names of company.
You can see by what was put on the invoice and initialed by the guy on the job my dissatisfaction.
