Massage Envy location or if he has a Dr. note stating her can no longer receive services (Please see the attached agreement that was signed by [redacted] for details). I called [redacted] on March 9th asking if he can call me back so we can get this situated and I have still not heard back from him. We also attempted to call him back in January and never heard back from then either (See attached screen shot for call notes). We try to work with our client as much as possible but if [redacted] doesn't return our calls then there isn't much we can do. He needs to fill out a cancellation form and provide proof of relocation or a Dr. note. Also [redacted] is not linked to this account in any way so she can not be the one to cancel the membership.
