To Whom it may concern:Re: Services rendered on 7/12/2013 Totaling $167.00 On the above mentioned date [redacted] received dental services which included dental x-rays, an examination and prophylaxis (cleaning). She had [redacted] dental coverage and a claim was submitted on her behalf. A...
payment was made by her insurance of $93.00 on 8/1/17, resulting in a balance of $74.00 not $80.00. That balance was probably due to a deductible and her services not being covered at 100%. That is an issue for her employer at that time who provided that benefit for her. We have no control over how services are covered. Four attempts were made to collect the balance including through [redacted] collection agency and finally in 9/ 2015 the balance was written off as uncollected. We are obligated to submit all services rendered to the insurance carrier. We are also obligated to perform a comprehensive service on new patients and this was done. It is unethical to advise patients or agree that we would only provide services that are fully covered even though some patients desire it. We are in the business providing a quality health service which explains why [redacted] is no longer in our care. We will attempt to locate the estimate of benefits (EOB) which is a document from the carrier which will outline the coverage of that date of service in more detail.
