Dear Ms***:
This letter is in response to Ms*** ***’s complaint received from the Revdex.com regarding his membership with the Louisville
Athletic Club III, LLC Ms*** originally signed her membership agreement on June 20, and, therefore, agreed to all the terms stated in the agreement.
According to the signed membership agreement, Ms*** joined on a month membership with an obligation date of June 30, 2013. As stated in the agreement, “This membership will automatically renew on a month to month basis upon expiration of the membership term.” Additionally, the agreement states that “a thirty (30) day advance written notice prior to the beginning of the next billing cycle is required to cancel membership, after fulfilling the original term of the agreement.” It is not our business practice to accept verbal cancellations. Our records do not indicate we have received the required written cancellation request. Based upon receipt of this complaint to the Revdex.com, I will accept this as your written notice. I will attach a copy of Ms***’s original signed membership agreement for reference.
Ms*** has a past due balance of $which is for December, and January, membership dues of $plus two late fee of $eachThis balance is rightfully owed per the signed membership agreement. In an effort to resolve this matter, if this payment is made by February 28, 2015, the February payment of $will not be owed. This is an exception as we are not applying our standard thirty day notice policy, which would require February to be owed also. If the $payment is not received, however, we will proceed with our collections process
