8/1/14 - 10/31/2014 carried $30.00 copay per insurance. For policy [redacted] with a coverage period of 11/1/14 - 10/31/15 carried no copay.The patient’s husband, [redacted] sent in a letter dated 8/21/15 asking for a $60.00 refund for copays that were paid for date of service 8/6/14 and 7/29/15. I called the insurance to verify what [redacted] stated in the letter. At that time the insurance explained about the different policies. I called and left a message for the patient to call me so I could explain why they were only going to receive a $30.00 refund. I never received a call back.
