This is an unacceptable response! The dental office/billing department have had an entire month to investigate this claimWhy are you just starting to investigate my Revdex.com claim now? I asked *** from the billing department to send me a written explanation of the billing along with the return of my financial documents (social security proof of income)Why haven't you honored my demand? I already paid the dental office the $fee for x-rays that I was promised by the dental office so again I will ask you, why do you continue to send me bills for an outstanding balance of $when in fact I should be paid in full? Why haven't you returned my financial documents?

I am writing in response to the query as to why Mr*** did not qualify for Long Valley Health Center's Sliding Scale FeeMr***, while he did provide us with an income verification, did not fill out the rest of the application for the sliding scale feeIn addition, it was not returned to us within days of being seen by a providerBoth of these requirements are part of our Sliding Scale policyWe would be happy to see Mr*** again and at that time if he would like to apply for the sliding scale fee as per our policy we would absolutely review it for that visit and any moving forwardUnfortunately it would not be retroactive to the visit currently in questionI have also mailed back his original income verification document to the address we have, *** on November 12, I hope this settles the matter, and explains why we were not able to offer Mr*** the sliding scale fee

***, the dental receptionist, promised me a low income sliding scale discount over the phone*** asked me how much money I make on Social Security and I told her the amount over the phoneThen she asked me to mail her documentation of my low income which I promptly didI was never told about forms that I needed to fill outWhy didn't the dental office mail me these forms immediately after I supplied the income verification? Bait and switch fraud!
