Patient was seen in our office on 11/11/for new patient comprehensive exam and cleaningAt time of appointment she discussed with Dr that LL tooth had been causing her sensitivity for approximately weekRadiographs were taken and tooth tested negative to all testsThere were no visible cracks or fracturesDr conservatively recommended to give the tooth a little time to calm downPatient was asked to call back for follow up appointment if it is not better within two weeks The patient did not call again until 3/21/early morning and spoke to our front deskShe stated she again had discomfort on LL tooth and would like it looked atPatient was added to our Dr’s schedule at 5:pm that same dayWe also discussed her request regarding the credit on her accountAfter investigating we found that the credit had been mailed Patient was seen in the afternoon on 3/21/where diagnostic x-rays were taken and all diagnostic tests performedAgain, both were negative and so he gave the most conservative option of having tooth adjusted and check back in a week The following morning, we spoke to patient and offered her to come in and see the owner dentist and let him evaluateShe was unable to make the appointments offered due to workShe had made an appointment at another dental office and would have them check tooth We asked her to please contact us after the appointment with the other provider to discuss their findings Thank you, DrMichael B [redacted] DMD, PC [redacted] @yahoo.com Brande H***, Mgr [redacted] @gmail.com Lifetime Dental Care phone Queensgate Dr fax Richland, WA

cracks or fractures. Dr conservatively recommended to give the tooth a little time to calm down. Patient was asked to call back for follow up appointment if it is not better within two weeks. The patient did not call again until 3/21/17 early morning and spoke to our front desk. She stated she again had discomfort on LL tooth and would like it looked at. Patient was added to our Dr’s schedule at 5:30 pm that same day. We also discussed her request regarding the credit on her account. After investigating we found that the credit had been mailed. Patient was seen in the afternoon on 3/21/17 where diagnostic x-rays were taken and all diagnostic tests performed.
