I am the beneficiary of the
insurance planLiberty has been telling my Medicare provider *** that payment has been made whereas it is totally wrongEven Liberty's officer Matt confirmed to me on 9/26/that check is mailed on that day after *** contacted him in this connection. Liberty should note that if provider " *** *** ***" is not paid by them, *** *** *** is asking me to pay for cleaning as per agreement signed. WHY LIBERTY CAN'T RETRIEVE WRONG PAYMENT MADE TO *** *** ***, ERIE, PA and MAKE CORRECT PAYMENT TO *** *** *** in Glen Allen, VA? What is holding them to do that when they agree that payment is made to wrong party?The attached document shows invoice sent by *** *** *** to me for payment and they harass me every now and then for paymentI curse the day when I went for cleaning under this insurance planPage shows Dental Claim Form submitted by Twin Hickory on 4/20/with claim # ***It no where shows claimer's name as *** *** ***, Erie, PA whom Liberty sent the check # *** dated 5/15/whose copy attached on page 3.It is very simple matter which Liberty is not resolving in amicable manner.

I did as I was advised by a service representative by the name of [redacted]. He works for Liberty Guardian and he advised me to call Blue Cross and file a complaint. this was done. TO date there has been no response from Blue Cross.
