On May 2, 2018, I booked an appointment with Larissa Lake and Coof *** *** for microblading with Stephanie L*** I was told over the phone that should I cancel the appointment or not show for the appointment, the deposit was non-refundable The deposit was $200, and I put it on my *** card The bill came through as $which included a service charge of $and tax, neither of which I was told of nor was either extra charge authorized
I arrived on time for my appointment which was scheduled for 6:PM on Thursday, June I was asked to complete a number of forms asking for quite a bit of medical information In addition to the medical information, the forms contained the following two clauses:

I agree to reimburse each the practitioner, Stephanie L*** or Sarah G***, and Larissa Lake & Cofor any attorneys’ fees and costs incurred in any legal action I bring against the technician Stephanie L*** or
