equipment in good working order, and I am found to be negligent, I will assume financial responsibility for repairs or replacement.” At the time Mr. ** was also provided a box and a pre-paid FedEx label in order to return the device. Mr. **’s final billing date was 8/31/2015, however Lantz Medical was not made aware of this until 10/21/2015. On 11/16/2015 Mr. **’s sales rep reported that he had been unable to reach him via phone to remind him to return the device. On 12/22/2015 after the rep stated that they had reached out to Mr. ** with no response, Lantz Medical sent an Inventory **tter stating that as a reminder the device belonged to Lantz Medical and if not returned that it would be invoiced to Mr. **. On 1/23/2016 Lantz Medical attempted to contact Mr. ** to once again remind him that the device needed to be returned. There was no answer so a message was **ft. On 3/7/2016 – Just over six months after Mr. ** was finished using the device and with it not being returned to Lantz Corporate (using a prepaid FedEx label and provided box), Lantz Medical sent an invoice to Mr. ** for the unreturned device. Mr. ** seeks final resolution for Lantz Medical to cease all collections attempts and clear the outstanding balance. Lantz Medical will agree to do that upon receipt of the device.
