Our Office has re-submitted the lab order in question back to the hospital on 6/12/& 6/26/with the additional diagnosis code requesting hospital to re-bill to patient's Insurance Company

In response to complaint #*** on June 12, 2017, for date of service on April 6, 2017, the patient was seen in the office for an annual physical/follow upHe was charged a co-pay of $which was paid at the time of serviceHis insurance was billed for the visit and subsequently has been paid
At that visit he was provided with a form for outside lab workThere is no billing from this office for the disputed amount of $

that dr. coded the lab work incorrectly that is why I was billed 360.00. I tried to get them to correct the mistake and they (s[redacted]) would not do it. the ins. co said it was his mistake and I would be refunded my 360.00 if he corrected his coding error. he coded the lab work as a diagnostic check for hypo thyroidism. not an annual physical. he did the coding. its his mistake.
